As we work together to navigate the COVID-19 pandemic, Middlesex Health is now offering Virtual Visits—also called telehealth or telemedicine visits. This will allow you to have certain types of appointments using your MyChart account on your phone, tablet, or computer.
Middlesex Health Primary Care offers many different types of virtual appointments, including new patient visits, sick visits, follow-ups, care management visits, and Medicare Wellness Visits.
Runways, Middlesex Health's new chronic care management program, is specifically designed to help patients with multiple chronic conditions such as diabetes, hypertension, obesity, and more. Runways provides guidance in between doctor visits to ensure you stay on course with managing your conditions.

A Patient Centered Medical Home (PCMH) is a primary care practice that combines teamwork and technology to provide evidence-based care that meets your unique health care needs and goals.
At a PCMH, you are the most important member of your health care team. You and your providers will work together to build a positive and productive long-term relationship. It also means that your primary care office is coordinating with other health care providers you might see. This leads to better care, a more positive experience and lower costs for you and your family.
A PCMH designation must be earned. The National Committee for Quality Assurance (NCQA) only recognizes a practice as a PCMH if they meet rigorous standards for high-quality, coordinated, patient-centered care.

If you have Medicare Part B, you are eligible for an annual wellness visit with Middlesex Health Primary Care.
A Medicare Wellness Visit is focused on preventive health care. This means that you and your doctor will work together to identify health risks and put together a plan for how to reduce them.
