Tribute Gifts

A tribute gift honors someone who is celebrating a special occasion, birthday, anniversary, special event.  Make your tribute now. 

If you would prefer to mail your gift, please fill out the Printable Donation Form and mail to Middlesex Hospital, Office of Philanthropy,  28 Crescent Street, Middletown, CT  06457.

When you make a tribute gift, if you so choose, we will send a personalized letter to the person you designate, notifying them that a gift has been made to Middlesex Hospital in their honor.  The amount of your gift is not given.  You will receive an acknowledgement from us for your tax purposes.
