Employee Health Screening Requirements

All applicants who have accepted a job offer with Middlesex Hospital are required to have a medical history interview and receive medical clearance by Occupational Medicine and Employee Health Services before attending Orientation and before beginning work. This process may take up to 2 weeks to complete. Please plan accordingly as non­compliance will delay your ability to start work.

Please review the following so that Occupational Medicine and Employee Health Services can medically clear you as quickly as possible.

  • After receiving an offer of employment, call Occupational Medicine at 860-358-2750 as soon as possible to schedule your appointment.
  • Please be prepared for your appointment by reviewing the medical requirements below. 
  • Complete the Comprehensive History Exam and bring to your appointment. 
  • Complete the OSHA Respirator Medical Evaluation (if needed) and bring to your appointment.

Middlesex Hospital Medical Requirements for All Employees


Before your first day at Middlesex Hospital, Occupational Medicine and Employee Health Services require the following information:

Drug Screening:

A drug screen will be performed as part of the pre-placement medical clearance process.

Physical Exam:

For several positions within the Health System, a physical examination will be performed as part of the pre-placement medical screening process.

TB Screening:

  • Screening must have been completed within the past 12 months. We require documentation of:
    • A QFT (QuantiFERON – TBGold Assay) result Or
    • 2 Step PPD result.

If the PPD documentation is Positive:

You will need to get a QFT (QuantiFERON – TBGold Assay) done.

  • If Results are negative nothing further is needed.
  • If results are positive, a chest x-ray is required. If the results of the chest x-ray are negative, nothing further is required. If the results of the chest x-ray are positive, follow-up with further testing is required. Applicant cannot begin employment until documentation of follow-up is provided.

You Must Show Documentation of These Immunizations:

  • Rubella - Proof of one vaccine or proof of immunity by lab test.
  • Measles
    • Proof of two vaccines Or
    • Proof of two MMR vaccines Or
    • Proof of immunity by lab test
  • Mumps
    • Proof of two vaccines Or
    • Proof of two MMR vaccines Or
    • Proof of immunity by lab test
  • Chicken Pox
    • History of Chicken Pox documented by physician Or
    • Proof of 2 vaccines Or
    • Proof of immunity by lab test.
  • Flu vaccine – Proof of vaccination is only needed during flu season. This is usually December through the end of April.

Recommended Immunizations - especially for those with direct patient care:

  • Hepatitis B Vaccine
    • If already received three doses of Hepatitis B Vaccine, and no proof of immunity provided, a lab test will be ordered by Employee Health.
  • Tetanus Diptheria and Pertussis Vaccine (TDaP) 

This information can be provided to Employee Health Services:

  • On the day of your pre-employment appointment or
  • Faxed to Employee Health Services at 860-358-6238 or
  • Emailed to employee.health@midhosp.org

Please Note:

If you do not have complete immunization records, please inform Occupational Medicine when making your appointment.
