Membership Application Packet

Cancer Center Membership provides a comprehensive, multi-disciplinary team approach, and continuum of care for the cancer patient.   This will assure the highest quality of care and the best outcomes.

There are two ways for you to become a Member of the Middlesex Hospital Cancer Center.  As a member of the Medical Staff of Middlesex Hospital, you will automatically become an Affiliate Member of the Cancer Center.  For those members of the Medical Staff directly involved in the care of cancer patients, there is now an opportunity to become a Full Member of the Cancer Center.

Becoming a Full Member demonstrates a provider’s commitment to the care of cancer patients through educational activities and involvement in Cancer Center programs.  Application to full membership is open to physicians, Physician Assistants, APRNs, and Registered Nurses with either Oncology Certification (OCN) or Certification in Hospice and Palliative Care (CHPN).

If you have any questions, please feel free to contact us at 860-358-2072 or cc_membership@midhosp.org.

Since then, he has performed more than 1600 weight loss procedures with some of the best and safest results in the country.

As an outspoken advocate for the morbidly obese individual, Dr. Jonathan Aranow has achieved national recognition for his leadership role in securing insurance coverage for weight loss surgery patients as well as for the superb results he has seen in his surgical patients. He notes that following weight loss procedures his patients routinely experience dramatic improvements in health and self-esteem. Because of the known benefits of these procedures, Dr. Aranow has campaigned in the state and federal legislatures for expansion of insurance coverage for these life-saving surgeries.

Dr. Aranow is adept at all available surgical techniques including gastric bypass, LapBand™, Realize Band™, sleeve gastrectomy, and minimally invasive procedures. His exceptional outcomes and comprehensive multidisciplinary program, which includes dietary and behavioral counseling, have received national recognition and have earned certification as a "Center of Excellence" from the American Society of Metabolic and Bariatric Surgery.
