The Center for Chronic Care Management at Middlesex Hospital

That’s why the Middlesex Hospital Center for Chronic Care Management in Middletown, Connecticut, offers programs that assist patients with a diagnosis of asthma, diabetes, and chronic heart failure to live well with their conditions. The Center also offers childhood obesity and smoking cessation programs.
