Diabetes Care Management Program
The Diabetes Care Program is a patient care program for people with diabetes. It is a major resource for primary care physicians, diabetic patients, and the community as a whole. The program includes information, assistance and ongoing support to assist participants in the program to reach their diabetes self-management goals and improve their confidence in living with and managing diabetes. Meetings are by appointment.
There are fees for the Diabetes Self-Management Education appointments and group classes and Medical Nutrition Therapy services. Most insurance plans will cover this service. Financial assistance is available.
Diabetes Support Group
Middlesex Hospital's Diabetes Support Group provides a forum where community members and their families can obtain both information and support in their efforts towards successfully living with and managing diabetes. Different monthly topics spur interest and discussion among the group members. There is no charge. Please call the number in the contact section for more information. Newcomers are welcome!
The program is free, and meets the first Wednesday of each month from September to June at 10:30 a.m., on the ground floor of 80 South Main Street, Middletown.
Fit for Kids - Childhood Weight Management
Fit for Kids is a unique program that connects children with a dietitian care manager who works with the child and family to teach essential tools and set goals that help families live a healthy and active lifestyle. While not a diet or weight loss program, Fit for Kids emphasizes prevention through everyday healthy eating and physical activity. Individual and group sessions are by appointment
There is a fee for this program; some insurance plans will cover the complete cost. The care manager will assist with the billing process. Financial assistance is available.
