Chronic Heart Failure Care Program

The Chronic Heart Failure Care Program assists individuals with Chronic Heart Failure and their caregivers to manage symptoms, improve overall health, achieve a more active lifestyle and reduce the need for emergency room visits. The care manager works with you and your doctor to design a plan to meet your needs. One on one counseling services are available for symptom control, Emergency plan, diet changes, medications and physical activity. Meetings are by appointment.
