At the Middlesex Hospital Center for Chronic Care Management, we have the resources to put you on the best possible track for maintaining the quality of life you deserve.

Here in Connecticut, our Diabetes Care Management Program is a major resource for primary care providers, diabetic patients, and the community as a whole. The program includes information, assistance, and ongoing support for participants to reach their diabetes self-management goals. Certified Diabetes Educators provide problem solving strategies and skills to help patients improve their confidence and ability to make healthy life-style changes.

Through the Middlesex Multispecialty Group, we offer board-certified physicians and a support staff experienced in diabetes care.

We also offer a number of programs and support groups to help patients maintain a positive outlook and provide up-to-date information about their disease.

Middlesex Hospital's Diabetes Support Group provides a forum where community members and their families can obtain both information and support in their efforts towards successfully living with and managing diabetes.

For children at risk because of excess weight problems, there is our Fit For Kids program. The program connects children with a dietitian care manager who works with the child and family to teach essential tools and set goals that help families live a healthy and active lifestyle.
