The Middlesex Hospital Homecare Program offers skilled nursing services to meet the needs of all patients – from infancy to end-of-life care, in a patient’s home, assisted living facility, or other locations right here in Connecticut.
Specialized Home Nursing Care
We tailor services and individualize care to meet the needs of all patients. We offer nurses to help patients manage needs such as IV care, pain management, therapies and more.
Hospice and Palliative Care Program
A team approach to care, including help with symptom control, nutritional needs, personal care and emotional support. This includes a nurse who provides special education and communication to patients, their families, and the skilled nursing facility staff. Heart Smart Program
A comprehensive heart-health program that includes early recognition, medical care, patient tracking, and counseling for patients with heart disease.
Telemonitoring
Home-monitored blood pressure, weight, and oxygen levels are sent electronically to a central station staffed by trained nurses.
Behavioral Health Care
For patients who need help coping with problems that occur as a result of illness or disability.
Care of Mother and Newborn
A program for newborn and toddler patients and their mothers.
Home Safety Evaluations

Advanced care by a nurse certified in the specialty of wound care and healing.
Other Specialized Homecare Services
Certified Home Health Aides
To help with personal needs such as bathing, dressing, and personal hygiene. Our aides are also trained to help patients with certain exercises and to manage some in-home medical equipment.
Rehabilitation Services
We offer complete medical rehabilitation performed by occupational, physical and speech therapy professionals.
Medical Social Workers
Our medical social workers help to identify resources for patients' long-term care planning. They provide the support and education patients and their families need when they are facing difficult challenges.
Lifeline Program
You can rest easy with Lifeline. Lifeline is an emergency response program linked to a personal emergency button worn by the client and connected with the phone system. A simple press of the button alerts emergency responders that the client is in need of help.
Community Health
Middlesex Hospital Homecare offers ongoing blood pressure clinics, flu shots, health fairs and links to other community services such as Meals on Wheels, transportation and adult day care.
Homecare Payment Information
Our fees are covered by Medicare, Medicaid, private insurance or direct payment. In cases of financial need, we make every effort to adjust payment depending on the services and outside resources.
