Required Steps Before Your First Day

We are excited that you have decided to join Middlesex Hospital and look forward to meeting you on your start date.  Below is a list of items that must be completed before your first day.  Please complete each of these important steps to ensure you will be set up and ready to go when you arrive.

  • Schedule Pre-Placement Health Interview Appointment

On the Employee Health Screening Requirements tab, you will find instructions and contact information to schedule your appointment with Occupational Medicine and Employee Health Services.  Please schedule your appointment as soon as possible and allow yourself at least two (2) weeks prior to your Orientation date for required medical clearance.

It can take up to 2 weeks for delivery, so it's important to order them as soon as possible.

  • Complete New Hire Forms

On the New Hire Forms tab, click on the link provided to access the following forms. These forms must be completed and sent to Human Resources prior to your Orientation date. Please visit Human Resources after your Employee Health Services appointment to provide two (2) approved forms of identification and confirm receipt of all new hire forms.

    •  Employment Eligibility Verification (I-9) form including two (2) approved forms of identification
    •  Tax Forms
    •  Connecticut W-4
    •  Federal W-4
    •  Employment Record Form
    •  Badge Request Form
    •  Employee Pledge/Core Values
    •  Medical Clearance Requirements
    •  Direct Deposit Form

  • Complete HealthStream Pre-Employment Requirements

On the HealthStream tab, click the link provided to access HealthStream.  You are required to complete all assigned orientation activities on the HealthStream site prior to your Orientation Date.  You will be compensated for this training upon your start date.  Please note that you will need to use your new Employee Number (E#) assigned to you in your offer letter.  If you do not have a computer available to you, use of the MHS Library computers can be arranged to complete the mandatory learning activities.  If you need assistance, please call Human Resources at (860) 358-6380.  

  • Obtain New Employee Badge

You will find your Badge Request Form with the other New Hire Forms as mentioned above.  Please see the Department of Safety & Security’s schedule located on the New Employee Portal under the Additional Employee Resources tab so you can arrange to visit and have your employee badge and parking sticker issued to you. An appointment is not necessary.
