Middlesex Health understands that your protected health information is confidential. We are committed to protecting health information about you. We create a record of the care and services you receive. We need this record to provide you with quality care and to comply with certain legal requirements.

We are required by law to:

  • to maintain the privacy of the protected health information that identifies you.
  • give you notice of our legal duties and privacy practices with respect to protected health information about you. This notice applies to all of the records of your care generated by Middlesex Health.
  • follow the terms of the notice that is currently in effect.

Joint Notice of Privacy Practices
This notice describes how medical information about you may be used and disclosed and how you can get access to this information. Please review it carefully.

This document describes how we may use and disclose your protected health information to carry out treatment, payment or health care operations and for other purposes that are permitted or required by law. It goes on to describe the types of information we gather about you, with whom that information may be shared and the safeguards we have in place to protect it. This notice also describes your rights to access and control your protected health information.

Complaint Form 
If you believe your privacy rights have been violated, you may file a complaint with the health system or with the Secretary of the Department of Health and Human Services. To file a complaint with the Middlesex, fill out the enclosed form and send to Privacy Officer, 28 Crescent Street, Middletown, CT 06457 or email the complaint form to HIPAA_Privacy_Office@midhosp.org. All complaints must be submitted in writing.

Request to Amend Form
If you feel that medical information we have about you is incorrect or incomplete, you may ask us to amend the information. You have the right to request an amendment for as long as the information is kept by or for Middlesex Health. We may deny your request for an amendment and if this occurs, you will be notified of the reason for the denial.
