Patient and Family Advisory Council (PFAC)

We are looking for people who have been patients, or have had family members as patients at the Middlesex Health System, and who are interested in helping us to improve our services for you and your family. The Patient and Family Advisory Council (PFAC) is a group of patients, family, caregivers, and health system staff whose mission is to collaborate on opportunities to achieve the highest quality, safest, and most respectful and compassionate care for everyone served.

The goals of the PFAC are to:

  • Improve the patient and family experience
  • Promote patient- and family-centeredness and engagement, setting a standard for incorporating the patient voice in health care
  • Strengthen communication and collaboration among patients, family, caregivers and staff
  • Educate patients and family about the importance of taking responsibility for their own health and health care
  • Provide input for achieving the highest level of patient safety

The Middlesex Health System PFAC first met in 2012. In the past several years we have accomplished much, just a few examples include:

  • Guiding Middlesex Health System to a staff uniform policy so people can more easily recognize each staff member’s role.
  • Working closely with Nursing staff to create a new patient information and communication board in each patient room.
  • Serving on or presenting to numerous other committees (including the Board of Directors) to provide a patient and family member perspective that can influence decision making and process development.
  • Presenting at the Connecticut Hospital Association (CHA) about a series of checklists created to help guide people on being more engaged in their care and to better participate in their care in the hospital. This also led ultimately to a state wide PFAC conference and an ongoing statewide PFAC group.

MEMBERS will be expected to:

  • Attend monthly meetings at 5:30pm on the 2nd Wednesday of the month. There may also be additional meetings for special projects or committees.
  • Maintain appropriate and confidential handling of personal information.
  • Be positive and supportive of the mission of the committee.
  • Be comfortable speaking within a group and expressing honest opinions.
  • Respect group process and work in a cooperative way.
  • Be able to work comfortably with people whose backgrounds and experiences are different from your own.
  • Serve a term as outlined in the Middlesex Health System Patient and Family Advisory Council by-laws (initial term is 2 years, and may be renewed for subsequent 2 year terms).
  • Participate in activities such as special projects or committees, as needed.
