Middlesex Hospital Anticoagulation Clinic is an outpatient clinic dedicated to managing patients on warfarin. We use a point-of-care device to obtain immediate International Normalized Ratio (INR) results using a small sample of blood from a finger prick. We are then able to adjust our patients’ warfarin doses based on the INR and other factors such as changes in diet, medications, or health. Our team includes a medical director, nurse practitioner, clinical pharmacists, and administrative assistants.
- Referral Process: To become a patient at our clinic, ask the doctor who prescribed warfarin to fill out an Anticoagulation Referral Form and fax it to our office. Once we have the appropriate paperwork we will contact the patient to schedule their first appointment.
